Access, transfer and retention
Access is limited to Prof. Dr. Osman Kelahmetoğlu and the authorized secretary/assistant. Data are not shared with third parties for advertising, marketing or commercial purposes. The system is hosted in Türkiye. Legally mandatory disclosures to authorized public authorities are reserved. If no healthcare relationship is established, pre-evaluation records are retained for a maximum of 1 year; if healthcare is provided, applicable health-law retention periods apply.

I have been informed through the separate Information Notice. I explicitly consent to Prof. Dr. Osman Kelahmetoğlu processing the identity/contact, application and health data that I voluntarily provide for preliminary health evaluation, application management and necessary communication with me.
I understand that access is limited to the physician and authorized secretary/assistant, that data are stored on infrastructure located in Türkiye, and that they are not shared for advertising/marketing except where disclosure is legally required. I may withdraw my consent via info@osmankelahmetoglu.com.
