Why has Deep Plane Facelift Been Talked About So Much Lately?

In recent years, one of the most discussed techniques among facelift surgeries has been the deep plane facelift. Especially on social media and digital platforms, this technique is frequently promoted as the “most effective facelift method.”

However, the popularity of a technique in aesthetic surgery does not mean it is the right option for every patient. Therefore, it is necessary to correctly evaluate what a deep plane facelift really means and why it has been so prominent in recent years.

What is a Deep Plane Facelift?

Deep plane facelift is not actually a new technique. This method was first described in the early 1990s. The fundamental difference, unlike classic facelift techniques, is going under the SMAS layer and repositioning the suspensory ligaments that carry the face by cutting them.

With this approach, it is claimed that a more pronounced shaping can be achieved by intervening in the deeper structures of the face.

Why has Deep Plane Facelift Become Popular Again?

Although the deep plane facelift is not a new method, it has become popular again in the last 3-5 years. The most important reason for this is its increased prominence on digital platforms and its marketing with the promise of longer-lasting results.

According to my own approach, the popularity of a technique in aesthetic surgery does not mean it is the best option for every patient. While more conservative and less radical surgeries have come to the fore in recent years, we see that the indications for the deep plane facelift technique have been expanded and recommended to a much wider group of patients.

Clinical Approach: Is Deep Plane Suitable for Every Patient?

My Clinical Experience: I am not a surgeon who performs deep plane facelifts. This technique was specifically described for patients with advanced skin sagging, usually in the 50-60 age group. However, today, unfortunately, it is presented as the only surgery even for younger patients with minimal facial sagging.

In my own practice, I evaluate each patient separately. Because what matters in facelift surgery is not the technique used, but selecting the most suitable technique for the patient.

Is Deep Plane Facelift Really Better?

It is often claimed that the deep plane facelift technique provides better shaping and its effect lasts longer. However, this is not true for every patient.

In the deep plane techniques that have become popular today, it may be a matter of loosening the suspensory ligaments instead of cutting them completely. This situation raises questions about whether a deep plane is actually being applied.

In addition, since this technique involves intervention in deeper tissues, it appears as a more radical approach that may increase the probability of facial nerve injury in unsuitable patients.

According to my clinical approach, a deep plane facelift is not always better. In experienced hands, both deep plane and SMAS plication techniques have similar effectiveness and similar duration of effect. Therefore, it is not correct to say "deep plane facelift is better."

Who is Deep Plane Facelift Not Suitable For?

Deep plane facelift is not suitable for every patient. Especially:

  • Patients in the younger age group
  • People with minimal sagging in the face
  • Patients with good skin elasticity

Successful results can be achieved with simpler and more conservative techniques in this patient group.

Which Facelift Techniques Do I Prefer?

In aesthetic surgery, I am in favor of applying the simplest possible technique in the most correct way.

In my own practice, I prefer to apply:

  • In the 30-40 age range and patients with minimal sagging → Mini facelift with SMAS plication and limited dissection
  • In patients of older age and with significant deformity → Facelift with SMAS plication and wider dissection

Difference Between Deep Plane and SMAS Techniques

The main difference between deep plane facelift and SMAS plication is the depth of the surgical plane. However, in practice, results do not depend solely on the technique.

According to the globally accepted view, the difference between these two techniques in terms of effectiveness and longevity varies depending on the surgeon's experience and the method of application.

Therefore, the correct technique should be determined based on the patient. It is not correct to say that a single method is ideal for all patients.

What is the Right Approach in Aesthetic Surgery?

The most important principle in aesthetic surgery is choosing the most suitable and correct method for the patient. Planning and the surgeon's experience are more decisive than technique names.

In my own approach, I aim to provide a natural and balanced result for the patient by choosing the simplest and safest possible method.

Therefore, I believe that patients should value the evaluation of an experienced surgeon who will make the most correct planning for them, rather than focusing on technique names.
